Policies

By registering at Lambarri Dance Arts in Matawan or Bayonne, NJ, I understand I agree to the following policies:

I understand these policies are effective throughout my dancers enrollment at Lambarri Dance Arts. I will only be prompted to sign a new policy/waiver when any changes are made or updates are added to Lambarri Dance Arts policies.

INJURY RELEASE I the parent/legal guardian acknowledge and give, to my minor under 18, permission to participate in dance and other athletic performance-related activities for which they are registered with Lambarri Dance Arts, LLC which may include, without limitation, participation in weekly classes, workshops, Photo Week/Weekend, The Spring Gala, all Virtual events whereby I participate at a remote location, and/or events to be named subsequent to the date hereof. I hereby agree on behalf of myself and the student of whom I am parent/guardian (if applicable) to abide by the rules and regulations of the Lambarri Dance Arts, LLC. as they may be in effect. I hereby authorize the Dance Studio and its owners and instructors to obtain emergency medical treatment either through my family physician or any other appropriate emergency medical service which may be available in the event of a medical emergency. I recognize and agree that the Studio does not provide Medical insurance for the benefit of its students, and I agree to be responsible for any medical expenses which may arise in connection with attendance or instruction at the Studio while the student is on the premises of the Studio or while the student is under the care or control of Studio Personnel. I hereby release and hold harmless the Lambarri Dance Arts, LLC. and its owners, employees, instructors and agents from all liability which may arise in connection with attendance or instruction at the Studio, while on the premises of the Studio or under the care or control of personnel at the Studio. This release is also on behalf of the minor student registered at the Studio where applicable. I agree to sign and be bound by the Studio standard release form.

PAYMENT POLICY Tuition, costume fees, performance fees, and deposit fees all have due dates. Those who opt into monthly tuition installments understand that payments are due on the 1st of each month and are late after the 5th. Those who opt into 2 payment installments understand that the 1st installment is due upon registering and the 2nd installment is due on December 1st and late after the 5th. Those who opt to pay in full understand payment is due upon registration. All other fees are late after the posted due dates. A $25 late fee will be charged for any late payments. Dancers will not be permitted in class, until the account balance is paid in full, if a payment is more than 30 days late. A $30 bank fee will be assessed for a non-sufficient funds payment, as well as the corresponding late fee. It is each parents responsibility to be aware of fees due, schedule changes, etc. Tuition payments received in full at registration will be discounted by 10% of the total amount owed. Sibling discounts are as follows: 10% off the 2nd student, 15% off the 3rd student, inquire for more than 3 children. Discounts cannot be combined. There are no refunds for missed classes or if classes are cancelled due to any force majeure event including but not limited to inclement weather, pandemic, government action, war and protest. Tuition fees, deposit fees, costume fees, and performance fees are non-refundable. There are no refunds for payments remitted if you chose to unenroll your child at any point. I agree I will be responsible for my child's account and activity requirements.

DROP POLICY Written notice must be submitted to the studio by the 15th of the month, if dropping from a class or the program, or the following months tuition will be owed by the 1st of that month for those on a month to month payment plan. Fees paid prior to dropping, including tuition, deposit fees, costume fees, performance fees, etc. are non-refundable. Any dancers who choose to re-register after dropping the program must, once again, pay a non-refundable deposit.

MAKE UP POLICY I understand that my dancer is allowed a maximum of 4 make up classes per style, per regular dance season. If a dancers exceeds 4 absences for any given style/class make up options will no longer be available. There are no refunds for missed classes. I understand when enrolling in Lambarri Dance Arts, I am willing to prioritize dance in my schedule regardless of the age or level of my dancer. I acknowledge I am committed to the program my dancer is enrolled in and prepared for my dancer to attend weekly classes consistently (on average, no more than 4 absences per dance style, per season). I understand that any more than 4 absences in a season will be detrimental to the progress of my dancer.

SUBSTITUTE INSTRUCTION POLICY I understand that Lambarri Dance Arts reserves the right to hire substitute instructors as necessary when a Faculty member is unable to fulfill his or her assigned classes due to (but not limited to) sickness, emergency, travel, etc. Lambarri Dance Arts is not required to provide prior notice that a substitute will be utilized. All classes will go on as regularly scheduled during substitute instruction and follow curriculums specified directly by Lambarri Dance Arts. All substitute instructors are established professionals vetted prior by Lambarri Dance Arts.

CREDIT CARD INFORMATION I understand that I must have a credit card on file at Lambarri Dance Arts in order to confirm the students registration. I understand that unless I authorize Lambarri Dance Arts the use of this credit card for automatic payments, the card will only be processed should I not make my tuition payments by the 5th of each month. I understand that if my payment is late, the credit card will be charged, including the $25 late fee, on the 6th of the month to fulfill my payment requirements. I understand I have the option to make payments by credit card, check, or cash prior to the 6th of the month to avoid having my card, on file, charged. I understand that should I want to have my payments charged automatically, I must authorize, in writing, Lambarri Dance Arts to do so. Should I choose to stop automatic payments at any point, I must authorize Lambarri Dance Arts 2 weeks prior to the next payment due date.

VIRTUAL CLASS POLICY I understand that should the studio be forced to close temporarily due to any force majeure event including but not limited to pandemic, government action, war and protest, the program will immediately transition to virtual instruction, for which all usual payment policies and drop policies apply. I understand the costs for virtual classes are the same as those for in person instruction. Virtual classes will not be available on a regular or weekly basis otherwise.

PHOTO/VIDEO RELEASE I authorize, on behalf of my minor student, Lambarri Dance Arts, LLC. to use, reproduce, and/or publish photographs and/or video that may pertain to the student including the students image, likeness, and/or voice. I understand that this material may be used in various publications, press releases, on our website, and on social media.

If you do not wish to agree to our Photo/Video Release, please sign this waiver and then immediately send an email specifying that you do not agree to the photo/video release, at which point that email will serve as an addendum to your signed waiver. You will be bound to all other policies except the Photo/Video Release in that case.
